Lindisfarne Productions was so pleased to once again be asked to create Patient Experience Awards Videos for the fabulous team at Health Quality Alberta. The produciton for this year’s patient experience videos took our team from Grande Prairie to Fort McMurray.

We were honoured to speak to the fabulous people behind the initiatives that are helping to create quality healthcare experiences for patients in Alberta. This year, we spoke to the teams behind My Next Steps, the Edmonton Diabetes and High-Risk Foot clinic, The Alberta Virtual Pain Program, and the Calgary Zone Music Therapy program.

My Next Steps is a resource created by patients, for patients. The resource aims to make the transition from hospital care to home smooth and accessible by empowering patients to be actively engaged in their own discharge planning. It is easy for Healthcare teams to implement and easy for patients to use and understand. By helping to improve transitions in care, My Next Steps reduces the risk of readmission or return visits to the E.R.

The Edmonton Diabetes and High-Risk Foot Clinic is an innovative new approach to diabetes care. In the past, diabetes care was fragmented, requiring patients to visit separate practitioners and clinics for all of their needs: education, specialist appointments, wound care, foot and nail care, insulin pump training, and importantly, mental health support. This often led to lack of communication, creating barriers and burden for patients and families. The Edmonton Diabetes and High-Risk Foot Clinic consolidated all of these services under one roof, giving patients the care they desperately need in a streamlined, barrier-free way.

The Alberta Virtual Pain Program offers easy and timely access to care and resources for people living with chronic pain. Patients are able to self-refer and attend virtually, breaking down barriers that often emerge for those without primary care providers, those whose diagnoses are often misunderstood, rural populations with limited access to care, and those who just struggle to leave their homes. The program offers interdisciplinary care and evidence-based non-pharmacological resources, helping patients manage their pain.

The Calgary Zone Music therapy Program offers music therapy to patients across several sites in the Calgary zone, including patients in palliative, neurological, and critical care units. Music therapy is an evidence-based, non-pharmaceutical therapy that integrates well with other therapies traditionally offered within health care environments.
